Do as we have documented we do (and Apache does), printing the %D as
request time in microseconds, truncated to an integer value.

The pass bits of the oc's flags wasn't cleared before or'ing the new
pass on failing the busy object test in the ban lurker. This would
cause the pass not to be matched later, making the lurker spin on the
busy object.

07d488cb partially broke varnishlog,
since libvarnishapi did not properly check if both -c and -b were
given.

In addition, varnishd did not emit a tag at the start of a backend

Make it clearer that vcl_hash is called on lookup, pass and pipe.

read error' when tripping on first_byte_timeout, and 'http read error'
when tripping on between_bytes_timeout (used to be 'http first read
error' in both cases). This now matches current master.

This looks like a bit of forgotten debugging, left by mistake.
